Bilingual Housing Case Manager
The Bilingual Housing Case Manager provides case management and essential services to individuals and families at risk of or experiencing homelessness in Adams County and Weld County. The Housing Case Manager will provide individualized client support by helping each client develop a personalized service plan to address any barriers to obtaining and maintaining permanent housing. The Housing Case Manager will also perform community outreach to business owners, realtors, landlords, housing developers and other service providers to build strong relationships and identify new and existing opportunities to better assist clients in accessing resources, employment, supportive services, and housing opportunities in Almost Home, Inc.’s service area.

Next Step Coordinator
The Next Step Coordinator (NSC) will work directly with Adams County Public School Districts to identify, assess an enroll students and their families into the Next Step Program. The NSC carries a small caseload of families in scattered site units within the community and coordinates wraparound services – such as employment support, financial education, access to nutritious food, afterschool programs, and mental/behavioral health treatment - throughout the clients’ transition to permanent housing, and up to 6 months following homelessness diversion. In addition to these services, the NSC will provide the re-housing support and case management to prevent identified households from becoming homeless and to those that are residing in the Almost Home, Inc.’s Emergency Shelter.

Grants Manager

The Grants Manager is responsible for helping to secure, document, and track corporate, association, foundation, and government grants for Almost Home, contributing to an annual revenue goal of approximately $4 million. This position works in a high-volume, fast-paced environment. The Grants Manager works cross-departmentally to ensure alignment and funding of key initiatives for the organization. In addition, this position will aid the Development team with corporate sponsorship identification, recruitment, and retention
