Housing Navigation Center

Navigation Centers are a critical resource to end and prevent homelessness in Adams, and Southern Weld Counties. The Almost Home Housing Navigation Center provides low-barrier access to on-site resource navigation and assistance applying for public benefits and emergency assistance programs. Clients accessing the Housing Navigation Center receive personalized support to help address housing barriers such as a lack of personal identity documents, employment connections, housing counseling and education regarding their rights and responsibilities as a tenant and coordinated care to address basic needs and Social Determinants of Health.  Almost Home’s Navigation Center Housing Navigators will communicate, encourage, facilitate, and support each client’s progress and activities toward housing and stability.

Hours of Operation:

  • Monday— 9:00 am-3:00 pm

  • Tuesday— 9:00 am-3:00pm

  • Wednesday— 9:00 am-6:00 pm

  • Thursday— 9:00 am-3:00pm

  • Friday—9:00 am-12:00 pm

The purpose of Almost Home’s Housing Navigation Center is to:

  • Help households maintain their housing with supportive services;

  • Connects those experiencing homelessness with mental/physical health care, workforce development services, and other long-term resources;

  • Connect households to the Coordinated Entry System for access to Rapid Re-Housing and Permanent Supportive Housing programs;

  • Assist households in getting back into permanent housing as quickly as possible.

 

The Almost Home Housing Navigation Center provides the following services:

  • On-site resource navigation;

  • Assistance enrolling into public benefits programs;

  • Information and referrals to meet basic needs and address Social Determinants of Health affecting housing stability;

  • Coordinated-Entry assessments for Rapid Re-Housing and Permanent Supportive Housing programs;

  • Access to computer and printing for housing and employment search;

  • Refer households to the Respite Housing and Severe Weather Activation Program (SWAP) for hotel/motel vouchers as available and as needed.

  • Workshops to support access to and maintenance of safe, healthy and affordable housing.
