Almost Home is seeking to hire a Grants Manager.

 

The Grants Manager is responsible for helping to secure, document, and track corporate, association, foundation, and government grants for Almost Home, contributing to an annual revenue goal of approximately $4 million. This position works in a high-volume, fast-paced environment. The Grants Manager works cross-departmentally to ensure alignment and funding of key initiatives for the organization.  In addition, this position will aid the Development team with corporate sponsorship identification, recruitment, and retention. Our programs save lives and ensure that our community has a resource to turn to when someone is experiencing one of the most stressful situations in their lives. As an agency, we are committed to our mission to promote self-sufficiency and provide assistance to those experiencing housing instability. Almost Home is seeking to hire a Bilingual Housing Case Manager.

The Housing Case Manager provides case management and essential services to individuals and families at risk of or experiencing homelessness in Adams County and Southern Weld County. The Housing Case Manager will provide individualized client support by helping each client develop a personalized service plan to address any barriers to obtaining and maintaining permanent housing. The Housing Case Manager will also perform community outreach to business owners, realtors, landlords, housing developers and other service providers to build strong relationships and identify new and existing opportunities to better assist clients in accessing resources, employment, supportive services, and housing opportunities in Adams County. The Housing Case Manager assists tenants in the assessment and resolution of potential evictions and provides emergency assistance in the form of rental and/or utility financial assistance to prevent homelessness or resolve the housing crisis. The Housing Case Manager is responsible for developing housing stabilization plans with clients to help meet Almost Home’s overall mission to promote self-sufficiency and provide assistance to those experiencing housing instability.

Almost Home is seeking to hire a Bilingual Housing Navigator.

 

This position will work in a collaborative effort with Broomfield County and partner agencies to implement Street Outreach and Severe Weather Activation Program (SWAP) for individuals and families experiencing homelessness. 

 The primary goal of SWAP is to provide temporary emergency shelter to unsheltered persons during periods of inclement weather and to provide re-housing case management to households experiencing literal homelessness. The primary goal of the Street Outreach program is to identify unsheltered homeless neighbors through direct street outreach activities and through community referrals from mental health, hospitals, local law enforcement and municipalities and other social service agencies. In identifying highly vulnerable homeless neighbors, the Bilingual Housing Navigator will administer assessment tools, facilitate placement into emergency housing, if needed and available, and connect to basic needs, community resources and permanent housing.

Housing Navigation Center Coordinator

 

The Housing Navigation Center Coordinator is responsible for the following: providing resource navigation services to drop-in clients; managing, expanding, and improving current Housing Navigation Center (HNC) programming; coordinating available community resources; and developing, implementing, and maintaining new programs.

The Housing Navigation Center (HNC) is a physical location at Almost Home, Inc. that provides low barrier access to on-site case management, supportive services, connection to services and a pathway to permanent housing through coordinated entry assessment. Clients accessing the Housing Navigation Center receive personalized support to help address housing barriers such as a lack of personal identity documents, employment connections, housing counseling and education regarding their rights and responsibilities as a tenant and coordinated care to address basic needs and Social Determinants of Health.

Almost Home is seeking to hire a Housing Navigator.

 

Our Emergency Shelter and Outreach programs are designed to be a soft place to land when someone experiences literal homelessness. In our Emergency Sheltering programs, we strive to help each family gain the resources they need to obtain and then maintain market rate housing once they move out. Our Outreach Programs are designed to provide supportive services, re-housing assistance and a warm, safe space to sleep during periods of inclement weather. Without these services, individuals and families would be living in places not meant for human habitation and we are doing our part as an agency to save lives.

 In addition, this position works directly with the Colorado Safe Parking Initiative (CSPI) to provide trauma-informed case management services to people who are experiencing homelessness in their vehicles, who have been provided initial outreach services, have been referred to a Safe Parking program and indicate a desire for additional services and the development of a case plan.
